Student Shadowing Disclosure Form

We have developed a disclosure form for use by members who have individuals shadowing them in an office setting. The form addresses confidentiality requirements should a shadow come into possession of information concerning the treatment and services provided by the doctor and his staff even though the shadow does not take any direct part in the treatment and services to the patient. The form delineates the responsibilities for confidentiality and intellectual property to which a shadow must adhere.
